I got mail from my health insurance, please provide me to the missing documents. I am attaching the same mail at the bottom of this mail.

Dear Sir/Madam,
We are in receipt of your claim documents (Claim no: 90258159—00) pertaining to Health Insurance policy (#[protected] and on evaluation, have found that the Following information/documents are not available with us.

PLEASE PROVIDE ALL INDOOR CASE PAPER RECORD WITH DOCTOR AND NURSE NOTES DURING HOSPITALIZATION.

PLEASE PROVIDE TEMPERATURE AND VITAL CHART DETAILS.
