Sir, i have started the policy for golden forests india ltd sinc 20/11/1996 due some unavoidable circumstances i could not deposit the premium. Now i wish to surrender my policy no. 01/9/rud79142 but the procedure could not be completed as the form required to be filled up. Whereas, correct amount deposited by me is not known by me.
