Adidas — Refund form

Hello. I have made an order from online store. I already returned the product and got an email from adidas team on 7th March 2022 but I have not received the form where I have to fill my bank account as I made a payment via COD. However I have received the tax credit note and I dont know where adidas will refund to since I have not yet received the form. Please solve this problem. I already try chat and call with the agents but the line were always buzy. Please help me.

Never received the refund or the form even though I returned the product few weeks back. My order number is AIN03105836.
