REQUEST FOR CHANGE OF ADDRESS

With reference to the above, I have to advise that I have applied for change address in the said ULIP plan as I have shifted from Kanpur to Lucknow. But regret to inform that my said reuest could not be done by UTI so far despite I have already deposited the necessary documents(KYC norms) i.e address proof. identity proof, PAN card, Bank statement verified from bank official in this regard.
Kindly look into the matter personally and the resolve my abovementioned problem at the earliest.
