Dear Sir,

I would like to inform you that I have purchased ULIP in 05.03.2001 of Rs.7,500/- for 10 Years. Subsequently I have paid 7 instalment of Rs.7,500/- per year from 2001 to 2007 and I have applied for redemption of all units in the month of December, 2007 till date I have not received redemption amount of Rs,96,650/- in December, 2007.

Further also note that in the meantime after redemption of TULIP in 2007 (as per your record) I have deposited one more instalment in 2008 for Rs.7,500/- and you have allotted fresh 458.626 units on 09.04.2008 in same ID No. I have applied redemption of fresh 554.938 (including Bonus) units in January, 2012 and received of redemption amount through ECS in HDFC Bank in February, 2012.

As per your record you had sent a cheque of Rs.96,650/- to my residence address but till date I have not received redemption cheque.

Kindly further check your record and arrange to sent my aforesaid payment of Rest.96,650/- with interest at the earliest
