I have purchased a Religare Health Care insurance from Policy Bazaar and the sales agent of policy bazaar has told me that they will provide IHO membership card and Rs.4000 consultation vouchers.

Its been more than 2.5 months and i have still not received the IHO membership card & consultation vouchers. Please find attached mail communication for your reference.
After continuous followup, then in this month they told me that they have dispatched the card via FLYMYPARCEL courier having shipment no as [protected], and when i enquired about the shipment through FLYMYPARCEL (Courier company) they told that the CITY mentioned in the courier is DELHI instead of PUNE.
My correct address is already updated in Religare Policy, but it seems they have mistakenly sent it to the incorrect address.
I am continuously following up with Policy Bazaar, but i still haven't received any concrete information.
Request your kind help in investigating and resolving this issue.
