I have lost my PAN card on24th May 2009. The no. is ACCPD0883G. What is the procedure for applying duplicate pan card?

I had submitted my pan card application no[protected] and processing fee coupon[protected]. I received my pan card yesterday. Surprisingly the name printed in the pan card is Abhilash Balachandran Krishnan Pillai. I Clearly mentioned in the column of your application form "THE NAME TO BE PRINTED ON THE CARD AS – ABHILASH BALACHANDRAN – I Dont under understand how you are printing such a long name in the card. You the people scrutinising the form illiterate or what? please correct the correct the card and send new one immediately without charging any fee. This is your mistake only.

mis printing of DOB in pan card.

sir i want to inform that there is misprinting of DOB in pancard.my dob is 15/09/1980 but in my pancard it become 13/09/1980. so please correct that as soon as possible.
